Periodic Oral Evaluation – established patient
This evaluation is performed to determine any changes in your dental and medical health status since your previous comprehensive or periodic evaluation. This includes an oral cancer evaluation, periodontal screening where indicated, and may require interpretation of the information acquired through additional procedures (such as periodic x-rays and charting) which are usually conducted at another time.
Limited Oral Evaluation – problem focused
This evaluation is specific to your oral health problem or concern. You may require additional diagnostic procedures (such as digital x-rays) for further diagnosis. These diagnostic procedures will help you and us plan for potential future appointments, a re-evaluation visit or other procedures.
Comprehensive Oral Evaluation – new or established
There is a more all-inclusive evaluation then the periodic exam. It is an evaluation and recording of the extraoral and intraoral hard and soft tissues, oral cancer evaluation, evident decay, missing or unerupted teeth, restorations, existing prostheses, occlusal (bite) relationships, periodontal (gum tissue) conditions as examples. This evaluation is intended for new patients or some established patients. Established patients who have had a significant change in health conditions, other unusual circumstances or absent from active treatment for three or more years would require this exam.
Detailed and extensive oral evaluation – problem focused, by report
A detailed and extensive problem focused evaluation which will include extensive diagnostic procedures to develop a treatment plan for a specific problem such as TMJ/TMD (temporomandibular joint/temporomandibular disease), facial pain, extensive chipped or worn teeth, bruxism/grinding and other dental irregularities.
Emergency (palliative) Treatment
This evaluation and treatment are based on a per visit emergency need to treat you, your dental pain and your immediate need for dental care.
